Appointments
We are accepting new patients of all ages. Please call our office to schedule an appointment.
Emergency appointments are available; however, we do not accept walk-ins.
Cancellations/Missed Appointments
As a courtesy to our patients, we will call to remind you of your appointment. Should you need to cancel or reschedule, we ask you to do so with 24 hours’ notice. At our discretion, we may assess a $50 no-show fee.
Weather policy
In case of bad weather, please call the office for an update. We rarely close due to snow.
Insurance
We are pleased to accept a variety of insurance plans. Please note that insurance information is subject to change. While contracted with these insurance companies, we cannot guarantee that we are in-network with your particular plan. We advise you to check with your plan before your appointment. Self-payment and co-pays are due at the time of service.
Please bring your insurance card and a photo ID to your visit. For insurances that require a referral, the patient is responsible for bringing the referral at the time of the visit.

- Kaiser
Prescription Refills
Please request a refill through your pharmacy. They will contact our office and seek insurance authorization if necessary. Requests are usually addressed within 24 hours—often the same day, but please do not wait until the last minute to order a refill.
If you are overdue for follow-up, you may be asked to schedule an appointment for an examination, allowing us to refill or adjust your medications or to try something new.
